To make sure that we have all the necessary information, please click the advance planning worksheet and complete as much information as possible and then hit submit. Finally, print the cremation authorization below, sign on the signature line (bottom, right) and then return by mail to: Cremation Basics, Inc., P.O. Box 914, Bangor, ME 04402.
